Medical Case Management
At Wits Solutions (WITS), we are the subject matter experts in providing complete medical case management services for military personnel who have medical readiness conditions that may affect their deployability or retention. We have a proven track record of delivering high-quality and timely services to our clients across the nation. Our services include but not limited to:
- Coordinating personnel's care from the identification of the medical readiness condition to the resolution of the case, either by achieving medical readiness or by transitioning to another program or status, such as medical retirement or separation.
- Requesting and coordinating profile exams, evaluations, and audiology exams at physical exam centers.
- Developing and implementing individualized treatment plans in accordance with relevant military regulations.
- Submitting and tracking case processing, medical documentation, and profile concerns to military or contracted providers.
- Attending medical readiness events in support of the state or national guard medical readiness mission.
- Confirming and following up on personnel referrals by providers and obtaining release of information from personnel when needed.
- Educating personnel on resources available to improve medical and dental readiness.
- Communicating and collaborating with delegated government personnel, unit commanders, health care teams, and personnel throughout the case management process.
- Utilizing military databases to validate, update, and document personnel's medical status and services.
- Ensuring personnel with medical profiles are case managed in the appropriate module.
- Performing administrative tasks in relation to state level injury processing and medical board processing in accordance with relevant military regulations.
Our medical case managers are qualified and experienced professionals who are committed to providing high-quality and compassionate care for our personnel. They are licensed vocational/practitioner nurses or registered nurses who have PC proficiency, specifically Microsoft Word and Excel. They adhere to the ethical principles and standards of practice of the case management profession and respect the rights and dignity of the personnel they serve. They also comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines.
